In this research we explain the configuration of some hormonal compounds and its effect of these configurations on the activity for example: Dexamethasone and Betamethasone, where the difference on the carbon number 16, where the methyl group take alpha position and beta position respectively where the beta is more active than dexa, estradiol and alfatradiol have Hydroxyl group at carbon number 17 in beta position and alpha position respectively, where the estradiol is natural estrogen while alfatradiol is in contrary to estradiol has no estrogenic effect, clomiphene have 2 configurations Z (cis) and E (trans), zuclophene and eneclophene, the enclophene isomer has antiestrogenic and the zuclophene has estrogenic activity, tamoxifen also has 2 configuration cis and trans which have estrogenic and antiestrogenic respectively, Alpha-Medroxyprogestrone is more active than Beta-Medroxyprogestrone in birth control, fluticasone and budesonide both are used in R and S epimers.
